Submission Guidelines
If you would be interested in adding "Your Story" to this section:

• Provide a maximum of 2,000 words of text in a Microsoft Word® file on disk, CD-ROM or as an attachment to an email.
• Provide any photographs you wish to accompany the text in digital form (minimum 200 dpi, less than 1.5 MB file size each). If your photos include patient settings, please provide only discreet photographs that protect the identity of the patient and any of their medical information.
